Poor maintenance under Railtrack led to a crash at Potters Bar that killed seven people in 2002.

Private contractor Jarvis originally blamed sabotage and later went bust. Network Rail took nine years to admit health and safety failures.

Four people died in a similar accident at Hatfield two years earlier. Railtrack had failed to keep safety records that could have helped stop it.
